Terms of Service & Privacy Policy

Thank you for choosing Integrated Care Unity Center (ICUC). Before your first session, please read and agree to the following Terms of Service and Privacy Policy.

Terms of Service

1. Therapy Services

 

ICUC provides therapy services that are designed to support your mental health and well-being. The nature of therapy requires active participation, and the effectiveness of therapy depends on your commitment to the process.

 

2. Payment for Services

 

ICUC operates on a self-pay basis and does not accept insurance. Payment is required at the time of your session unless a prior payment plan has been arranged. The fees for services will be discussed with you before your first session. We do offer super bills for client's that want to submit to their own insurance. 

 

3. Cancellation, No-Show Policy, and Late Policy

 

We require at least 24 hours’ notice if you need to cancel or reschedule your appointment. Cancellations made less than 24 hours before your appointment, or missed appointments without notice, will result in a fee equal to the full cost of the session. This fee is your responsibility and must be paid before scheduling your next appointment. If you are more than 15 minutes late, you have forfeited your appointment. You must call your counselor to reschedule. After two missed appointments in which you no- show or cancel, therapy services may be discontinued, and you will be referred to seek therapy with another agency or private therapist.  

 

4. Confidentiality

 

All sessions are confidential in accordance with HIPAA regulations. Information shared in therapy will not be disclosed to anyone without your written consent, except in cases where disclosure is required by law (e.g., there are a few exceptions to this like Child abuse, elder abuse, dependent adult, or harm to yourself or others.) 5. Communication

 

You may contact your therapist via phone or email for administrative purposes (e.g., scheduling, billing). Please be aware that electronic communication may not be fully secure. For privacy reasons, it is advised to limit sensitive information shared via email or text.

 

6. Therapeutic Relationship

 

The relationship between you and your therapist is a professional one. To maintain this relationship, it is important that interactions outside of the therapy context are avoided. Your therapist is here to support your mental health needs and will refer you to another provider if necessary.

 

7. Termination of Services

 

You have the right to terminate therapy at any time. Likewise, your therapist may suggest termination or referral to another provider if it is determined that services at ICUC are no longer beneficial or appropriate.

 

8. Governing Law

 

These terms are governed by the laws of Texas. Any disputes arising from your use of our services will be subject to the jurisdiction of the courts in Texas.

Privacy Policy

1. Your Privacy

 

Your privacy is of utmost importance to us. We are committed to protecting your personal and health information in compliance with HIPAA and other applicable laws.

 

2. Information We Collect

 

We collect personal information, such as your name, contact details, and health history, to provide effective therapy services. This information is collected through intake forms, direct communication, and during therapy sessions.

 

3. Use of Your Information

 

Your information is used solely for the purpose of providing therapy services, including treatment planning, scheduling, and communication. We will not share your information with third parties without your explicit consent, except as required by law.

 

4. Security of Your Information

 

We take appropriate measures to ensure that your information is stored securely and protected against unauthorized access. This includes using secure systems and limiting access to your information to only those who need it for treatment purposes.

 

5. Your Rights

 

You have the right to access, review, and request amendments to your health records. You also have the right to request restrictions on certain uses and disclosures of your information. If you wish to exercise these rights, please contact our office.

 

6. Electronic Communication

 

Please be aware that while we strive to maintain confidentiality, electronic communications such as email and text messages may not be fully secure. For this reason, we recommend that sensitive information be discussed in person or over a secure phone line.

 

7. Changes to Privacy Policy

 

We may update this Privacy Policy from time to time. Any changes will be communicated to you, and the updated policy will be available on our website and at our office.

 

8. Complaints

 

If you believe your privacy rights have been violated, you have the right to file a complaint with ICUC or the U.S. Department of Health and Human Services (HHS). Filing a complaint will not affect your care in any way.
